Before participating in the SlimShot360 Lipo Dissolve program provided by Epiq'ly Contoured Wellness Studio, it is important that you carefully read and understand the following release of liability. By proceeding with the program, you acknowledge and accept the terms outlined in this disclaimer:

1. Assumption of Risk: I understand that participating in the SlimShot360 Lipo Dissolve program involves certain risks. These risks include, but are not limited to, allergic reactions, bruising, swelling, infection, skin irregularities, and dissatisfaction with the results. I voluntarily assume all such risks associated with the program.

2. Acknowledgment of Responsibility: I acknowledge that Epiq'ly Contoured Wellness Studio, and their employees have provided me with guidelines, instructions, and information regarding the self-injection process, including where and how to inject the solution. I understand that it is my responsibility to follow these guidelines and instructions accurately and exercise caution throughout the program.

3. Release of Liability: In consideration of participating in the SlimShot360 Lipo Dissolve program, I hereby release Epiq'ly Contoured Wellness Studio, and their employees from any and all liability, claims, demands, actions, or causes of action arising out of or related to my participation in the program, including but not limited to any personal injury, loss, or damage that may occur as a result of the program.

4. Consultation with Healthcare Professional: I understand that it is recommended to consult with a healthcare professional before starting the program. I acknowledge that Epiq'ly Contoured Wellness Studio are not responsible for any pre-existing medical conditions, contraindications, or adverse effects resulting from my participation in the program.

5. Compliance with Laws and Regulations: I agree to comply with all applicable laws, regulations, and guidelines related to the use, disposal, and handling of injectable solutions, needles, and syringes during the program.

6. Personal Responsibility: I accept full responsibility for my own health, well-being, and decision to participate in the SlimShot360 Lipo Dissolve program. I agree to disclose any relevant medical history, medications, or conditions that may impact my suitability for the program.
